Complete Our Online Registration Form
Complete our secure online Patient Registration Form to preregister with our office. Your privacy and security are paramount, and we’ve taken all necessary precautions to safeguard your personal information. Upon your first visit, we’ll have your completed form ready for your signature.

Schedule Your Appointment
Our office hours are Monday to Friday, from 8:00 am to 5:00 pm. To schedule your appointment, simply call us at (731) 891-0881. Our experienced patient coordinators will assist you in finding a convenient time for your visit.

The Day of Your Visit
Before your appointment, please review our Pre-Operative Surgical Instructions. If you’re undergoing surgery, remember not to eat or drink anything for six hours prior to your appointment. Additionally, ensure that a responsible adult accompanies you to the office, stays during the procedure, and drives you home afterward. However, if your appointment is for consultation only, these requirements do not apply.

After Your Surgery
Following your surgery, experiencing some pain or swelling in the first 24 hours is normal. Refer to our Post-Operative Surgical Instructions for essential information on post-operative care and what to expect during your recovery period.
